- Certificate of Good Moral
Grade 8 / Grade 9 / Grade 10 and Grade 12

- Honorable Dismissal (for Transferee)

Higher Education
- For Scholarship, Please proceed to the Registrar's Office for detailed instructions and requirements (Annex Campus).
How to apply
Follow these simple steps to complete your enrollment
1. Get the Form
For College Applicant please register first by clicking the link Register Now!
Then get the enrollment form from the registrar's office (Annex) or download it online.
Skip this if Basic Ed
2. Submit Requirements
Bring the completed form and all required documents to the admissions office (Annex).
For Basic Ed (Main).
3. Subjects Encode
Bring your confirmed schedule to the main registrar for encoding, then proceed to the Finance Office to receive your printed assessment (Main)
For Basic Ed proceed to EVP office for sectioning and assessment (Main)
4. Pay Tuition
Proceed to the cashier to settle the enrollment fee and tuition payment. (Main)
